About the Job:
Caring Hearts Homecare is looking to hire an Intake Nurse to assist in the intake process and assist in day-to-day supervision of direct patient care.
Qualifications:ย 
  • Must have a current RN/LPN license in Massachusetts.
  • Background in public health, home health, or hospital work.
  • At least one year of clinical experience.
  • Hands-on nursing experience.
  • Home care experience preferred.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Good computer skills.

Responsibilities:
  • Assist in taking referrals and assigning patients to field staff.
  • Follows up on referrals by contacting patients, physicians, family members, etc.
  • Assist in handling patient care supervision and day-to-day patient care management.ย 
  • Assist in managing the HR needs of the clinical department, notifying the Branch Manager/Administrator when new hires are needed to stay proactive with the growth plan.
  • Assists with new hire screenings/ interviews of home health care personnel.ย 
  • Orients professional staff to clinical policies and procedures of our Agency.
  • Performs other tasks as assigned.
