Hospice Office Intake Coordinator

Reports To: Hospice Director / Clinical Manager
Location: Office-based

Job Summary:
The Hospice Office Intake Coordinator is responsible for managing all incoming patient referrals, coordinating admissions, and ensuring accurate and timely documentation to facilitate smooth patient care initiation. This role acts as the primary point of contact for patients, families, hospitals, and referral sources.

Key Responsibilities:

  • Receive and process new patient referrals from hospitals, physicians, and other healthcare providers.

  • Conduct initial eligibility screening for hospice services.

  • Schedule admissions visits with clinical teams.

  • Collect and verify patient demographic, insurance, and clinical information.

  • Enter all referral and patient data accurately into Electronic Health Records (EHR) or hospice software (e.g., MatrixCare).

  • Communicate with families and caregivers regarding hospice services, benefits, and admission process.

  • Coordinate with intake and clinical staff to ensure timely and compliant admissions.

  • Maintain accurate and up-to-date tracking logs and reports for all referrals and admissions.

  • Support billing and compliance by ensuring required documents are completed before patient start of care.

  • Provide exceptional customer service and maintain professional communication with external and internal stakeholders.

Qualifications:
