Location: Maitland, FL | Schedule: 40 hours/week, On-Call Rotation

Our growing home health agency is seeking a dynamic LPN Intake Coordinator to join our dedicated team in Maitland, FL. What You'll Do
As the LPN Intake Coordinator, you'll be the heartbeat of our office operations and intake process. Responsibilities include:
  • Answering phones and managing fax communications
  • Entering patient referrals into the EMR system
  • Assisting with coordination of home health services
  • Creating, modifying, and maintaining forms and records
  • Filing, distributing Medicare paperwork, and tracking physician orders
  • Assembling admission packets
  • Supporting quality assurance and regulatory compliance efforts
  • Jumping in to help your team whenever needed
Qualifications
  • Active LPN license (required)
  • Valid driver's license & reliable transportation
  • 1+ year of home health experience (required)
  • Experience scheduling home health referrals (preferred)
  • Proficiency with Microsoft Office Suite (Word, Excel, etc.)
  • Ability to participate in on-call rotation
