HOURS:Part-time (20 hours per week)

RESPONSIBLE TO:Director, Pine Bluff

DIRECT REPORTS:None

LOCATION:1305 E 6th St, Pine Bluff, AR

SALARY:$18.00 per hour, paid bi-weekly

    Position Description

    The Opportunity House project provides emergency housing for 35 single adults and families. Services are recovery-oriented, trauma-informed and participant-centered. The Case Manager is responsible for conducting assessments to determine client needs and plans for obtaining needed services. The Case Manager will demonstrate direct assistance, coordination of care, and linkage to the appropriate services while developing support plans for residents. The goal of the program is prepare clients for permanent housing.

    Primary Duties and Responsibilities

    Essential Functions
  • Outreach/screen potential clients for eligibility using the ESG guidelines for all referrals
  • Provide guidance and support for clients completing intake assessments, referral interviews, regular case management meetings, service plans, semiannual assessments and housing placement planning.
  • Establish goals and complete case notes on goal progression in New Org data system on a weekly basis, at minimum.
  • Meet with clients and complete intake paperwork in a timely manner for all new clients seeking case management or housing services.
  • Track client enrollment and prepare reports, statistics, and paperwork as requested by supervisory staff and required by grants that support case management.
  • Assess the needs of clients and refer them on to appropriate agencies and social service programs. Complete referrals for supportive services for Social Security, SNAP, housing, mental health assistance, health insurance, and other services. Monitor and assess connections to supportive services.
  • Provide comprehensive employment navigation services, including career planning, job search assistance, employer referrals, and employment retention support.
  • Facilitate at least one weekly group activity for program residents to build life skills, increase communication or promote recreation opportunities.
  • Mediate conflicts or issues between clients and/or between clients and staff in a constructive and empathetic manner, fostering a safe and respectful environment for all participants.
  • Ensure client files include required eligibility documents, progress notes, service plans, and other necessary paperwork
  • Assist the program team with special events or other activities
  • Provide medication, education and management
  • Assist clients in obtaining and maintaining earned income including attending job training, improving job skills, pursuing education (e.g., GED), and finding employment
  • Coordinate care with other service providers and involved supports
  • General Responsibilities
  • Stays informed of current local and national events, trends, and issues (wherever possible) that impact Depaul USA’s work with the homeless.
  • Strives continuously to develop the role in collaboration with the supervisor to ensure that all tasks are undertaken effectively and appropriately, meeting the strategic goals and objectives of Depaul USA.
  • Participates in internal/external meetings as required and attends training events, conferences, and other functions as necessary. Participates in regular supervision and annual appraisal and helps in identifying your jobrelated development and training needs.
  • Ensures adherence to all Depaul USA policies and procedures, particularly those relating to Health and Safety, Safeguarding, Code of Practice, and Confidentiality.
  • Contributes to the effective implementation of Depaul USA’s Equal Opportunities Policy as it affects both the organization and its work with this client group.
  • Undertakes the role professionally, always maintaining a highquality work standard and always working in accordance with the Vision, Mission, and Values of Depaul USA.

  • Qualifications

    The ideal Case Manager will be a professional with at least one year of experience in case management with vulnerable segments of the population. S/he will have experience working with individuals who have faced homelessness and struggled with issues of mental illness, cognitive disabilities, and addictions, as well people who are newly housed or employed. The Case Manager should possess the following:

  • Minimum of an associate’s degree in related field (ex. social work, psychology, etc.); 1 year work experience in related field
  • Demonstrated knowledge of core social work principles, ethics, and case management practices, including assessment, service planning, documentation, resource coordination, advocacy, and maintaining appropriate professional boundaries.
  • Exceptional communication, multi‐tasking, conflict management and interpersonal skills.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint), and experience with data management systems
  • Ability to organize tasks, events, people, and delegate effectively.
  • Ability to exercise independent judgment when appropriate and strong decisionmaking skills.
  • Familiarity with traumainformed care, harm reduction models, and strengthsbased approaches to service delivery.
  • Experience working with diverse populations
  • Work Schedule:
  • Onsite at Opportunity House with limited offsite work.
  • Schedule: MondayFriday, between 7:00 AM to 11:00 PM CST.
  • Must be available to work occasional evenings and weekends as needed.
