Monday - Friday 8:00am - 5:00pm


The Intake and Assessment Specialist I completes the initial intake and behavioral health assessment process for all incoming OPCS clients, requiring strong clinical and critical thinking skills, a firm grasp of OPCS programs, and knowledge of diagnoses, particularly substance use, ASAM assessments, and crisis planning. The role involves interacting with individuals who are experiencing homelessness, Serious Mental Illness, Substance Use Disorder, and other related conditions.

Duties and Responsibilities:


1. Complete Behavioral Health Assessments, write Recovery Plans, and discharge summaries. 

2. Update Recovery Plans as services are completed or if it is determined that the client needs services outside of the current plan.

3. Complete suicide risk screenings and plans.

4. Document services and information within the Electronic Health Record (EHR).

5. Maintain all client files in a confidential manner and in compliance with HIPAA standards. 

6. Engage in clinical supervision with Clinical Supervisor.

7. Prescreen client readiness to enroll in group and individual services. 

8. Review and Complete received Referral Paperwork for clinical individuals and group services.

9. Connect/Schedule clinical direct service staff with referred clients to initiate individual and group services.

10. Other duties as assigned.

Requirements

1. Must meet one of the following education and experience requirements:

a. Master's degree or bachelor's degree in a field related to behavioral health. 

b. Bachelor's degree (in any field) and at least one year of full-time behavioral health work experience 

c. Associate's degree and at least two years of full-time behavioral health work experience. 

d. High school diploma or high school equivalency diploma with at least four years of full-time behavioral health work experience. 

2. Demonstrated competency in Evidenced-Based Practices including: 

a. Housing First

b. Harm Reduction

c. Motivational Interviewing

d. De-escalation (CPI Model)

3. Possess and maintain valid CPR/First Aid Certification, or ability to complete training and pass certification.

4. Knowledge of resources in the community. 

5. Strong communication and interpersonal skills.

6. Strong computer skills (e.g., Microsoft Word, Outlook, Excel).

7. Self-starter committed to a team approach, responsible, dependable, ability to set priorities, meet deadlines and work flexible hours.

8. Experience using an Electronic Health Record (EHR).

9. Valid Arizona driver's license, proof of auto insurance and reliable transportation.

Pay Rate: $20.57 - $25.21 - $29.86 

In accordance with pay transparency, the salary reflected in this posting is the full salary range for this position. Individual placement within the range is based on the candidate's current experience, education, skills, and abilities related to the position. Salary placement is typically between the minimum and mid-point of the salary range.
