Requirement: Bachelor's Degree in Social Work/Human Services
The Social Worker is responsible for providing services to meet the social and/or emotional needs that affect the residents’ ability to achieve their highest level of function; participate in the development of residents’ comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations. Tasks include but are not limited to:
  • Participate in the resident’s interdisciplinary care planning process.
  • Perform resident assessments of baseline, current medical condition, current disposition needs, and on-going monitoring of residents’ functional and medical needs.
  • Conduct MDS reviews timely including face-to-face cognition and mood interviews with the residents.
  • Document psychosocial needs, interaction and follow-up actions with residents and families according to facility policies and procedures.
  • Attend weekly meetings to review current status of short-term residents
  • Maintain departmental documentation
  • Provide Notice of Non-Coverage and ensure prompt signature of responsible party
  • Facilitate documentation for resident’s DME and home health or facility referrals
  • Facilitate the discharge of residents when needed. This includes discussing options, providing resources, facilitating referrals and coordinating with community representatives.
  • Ensure that the residents’ therapeutic, nursing, and level of care needs are appropriately met by the discharge plan.
  • Provide on-going psychosocial well-being support to the residents and families as needed. This may include visits to the residents, follow-up with psychiatry services, and follow-up or visits with the family.
  • Provide quality service to all residents and guests; remember that Hillcrest means Elegant Care
  • Maintain awareness of surroundings, including observing residents within eyesight, being alert to their needs; summon assistance from Nurse Supervisor or other staff as needed
  • Perform other duties as required by the Administrator.
