Job description

Serenity Hospice is seeking an MSW for our Portland Metro team.

What Youll Do:

  • Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patients plan of care. Provides an assessment in the patients identified residence and assistance when this is not safe, and another plan is required
  • Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
  • Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
  • Provides accurate and timely documentation of patient services to reflect the plan of care
  • Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
  • Serves as liaison between patients and families/caregivers and community agencies

Qualifications:

  • Social Work Degree
    • Idaho: Bachelors degree in Social Work; Masters degree preferred.
    • Illinois: Bachelors degree in Social Work; Masters degree preferred.
    • New Mexico: Masters Degree in Social Work required.
    • Ohio: Bachelors degree in Social Work; Masters degree preferred.
      • The bachelors degree can be in Psychology, sociology, or other field related to Social Work.
    • Oregon: Masters Degree in Social Work required.
    • Pennsylvania: Bachelors degree in psychology, sociology, or other field related to social work; Masters degree preferred.
      • Bachelors degree must be supervised by a full-time MSW and have at least 1 year of social work experience in a healthcare setting.
    • Tennessee: Master's Degree in Social Work required.
    • Texas: Bachelors degree in Social Work; Masters degree preferred.
    • Virginia: Bachelors degree in Social Work; Masters degree preferred.
  • Current State Licensure in Social Work
    • Idaho: LSW, LMSW, or LCSW
    • Illinois: LSW or LCSW
    • New Mexico: LMSW or LCSW
    • Ohio: LSW, LISW, or LPSW-S
    • Oregon: Licensed not required.
    • Pennsylvania: LSW or LCSW, preferred
    • Tennessee: LMSW, LCSW or or LAPSW can practice under the supervision of an LCSW
    • Texas: LBSW, LMSW, or LCSW
    • Virginia: Licensed not required.
  • Minimum of one years experience in health care or hospice setting
  • Valid drivers license and proof of insurance is required
