We are currently seeking a PRN Registered Nurse (RN) Case Manager to join our team. The Registered Nurse (RN) Case Manager will be responsible for providing comprehensive case management services to our clients, ensuring they receive the highest level of care and support. Responsibilities of a Registered Nurse (RN) Case Manager:

  • Complete new client admissions, including conducting initial assessments, obtaining medical histories, and developing individualized care plans.

  • Provide education and support to clients and their families on health management, medication management, and safety practices.

  • Participate in case conferences and team meetings to discuss client progress, identify areas for improvement, and develop strategies to enhance care delivery.

  • Act as a liaison between clients, caregivers, and other healthcare professionals to facilitate communication and ensure the highest level of care and support.

  • Coordinate and oversee the implementation of care plans, including coordinating with other healthcare professionals and community resources as needed.

  • Provide ongoing monitoring and evaluation of client progress and make adjustments to care plans as necessary.
