Yakima Family Health Services is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our team in Goodyear, Arizona. This full-time, remote role offers a competitive salary range of USD 87,000 – 105,000 annually. This position is primarily remote, allowing flexibility in managing your schedule while engaging with patients through telehealth services and virtual appointments. • Conduct comprehensive patient intakes consisting of diagnostic and functional evaluations, making informed recommendations to primary care providers regarding treatment goals and plans.
• Provide brief, focused therapeutic interventions for patients requiring mental health support, ensuring alignment with Agency and Federally Qualified Health Center (FQHC) guidelines.
• Initiate follow-up measures to monitor patient progress, adjusting treatment approaches as necessary to meet their evolving needs.
• Assist primary care providers in recognizing and managing mental health issues, guiding patient referrals when specialized care is necessary.
• Document all patient interactions, progress, and diagnostic information accurately in treatment charts, maintaining clear communication with primary care providers.
• Develop and implement relapse prevention plans for patients, promoting stability and well-being in their daily lives.
• Offer educational resources and self-management strategies to empower patients as active participants in their recovery journey.
• Consult with clinic management and interdisciplinary team members regarding behavioral health services and program evaluations to drive improvements in patient outcomes.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals and fulfill other duties as assigned.

In this role, you will support a diverse patient population, addressing a range of mental health needs impacted by socio-economic challenges, cultural backgrounds, and family systems. • A Master’s degree in Clinical Counseling from an accredited institution.
• A current and valid LPC (Licensed Professional Counselor) license is mandatory.
• Demonstrated knowledge in mental health assessment, treatment planning, and psychotherapeutic techniques.
• Ability to evaluate individual patient needs to provide tailored care effectively.
• Proficient interpersonal skills to collaborate seamlessly with healthcare professionals and engage patients meaningfully.
• Strong documentation skills to maintain accurate clinical records and facilitate quality care.
• Ability to navigate interdisciplinary team settings, ensuring a harmonious workflow.

• Experience in providing counseling services within outpatient settings or community health organizations.
• Familiarity with DSM-IV diagnoses and behavioral therapy practices.
• Bilingual proficiency in English and Spanish is preferred, enhancing accessibility for our diverse client base.
• A strong understanding of community resources and systems that support mental health initiatives.
