About the Role - Case Manager
The Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.
What You'll Be Doing
  • Conduct assessments to determine clients' needs and develop individualized care plans
  • Coordinate and monitor services to ensure clients' needs are met effectively
  • Provide crisis intervention and support during challenging situations
  • Guide and supervise clients to promote personal growth and development
  • Utilize motivational interviewing techniques to empower clients towards positive change
  • Collaborate with community resources and agencies to enhance service delivery
  • Assist with workforce development initiatives for clients seeking employment opportunities

What We're Looking For
