Job description

SUMMARY:
Formulates, selects and delivers medications in accordance with prescriptions and ensures the safe and effective use of pharmaceutical within the institution.
ESSENTIAL FUNCTIONS (under direct pharmacist supervision):
Interprets and clarifies written orders for all types of medications.
Compounds, labels and packages medications and pharmaceuticals.
Secures storage of narcotics and other controlled substances.
Provides medication and drug information relative to circumstances and patient conditions
Communicates drug-related side effects, dosages, and/or potential complications to health care professionals.
Clarifies orders for all types of medications.
Complies with Federal and State statutes, rules and regulations regarding the practice of pharmacy.
Prepares and makes deliveries to patient care areas and ancillary units.
Documents pertinent information in a clear, complete, concise and organized manner.
Replenishes automated dispensing units/ pharmacy floor stock for nursing units
Counts, packages and labels unit doses and fills routine orders for inspection by pharmacists.
Uses aseptic admixture technique in the horizontal or vertical laminar airflow hood and procedures to minimize contamination of sterile intravenous products in accordance with USP Sterile Compounding standards
Enters or retrieves data from the electronic medical record.
Advises patients and/or their representatives on use of various medications, including dosage, side effects and directions for use.
Remains informed about current developments concerning new drugs or test results from research laboratories regarding drug usage; researches drug information requests and responds promptly.
Promotes effective use of formulary system through drug product selection and therapeutic alternative product suggestion for non-formulary orders.
Monitors drug therapy and suggests alternative drug therapy when appropriate.
Consults with and advises medical staff personnel regarding utilization of drugs and pharmaceuticals.
Adheres to TMCH organizational and department-specific safety, confidentiality, values policies and standards.
Assists with the medication reconciliation process by updating the prior to admission medication list.
Performs related duties as assigned.
MINIMUM QUALIFICATIONS
EDUCATION: Currently enrolled in an AACP-approved College of Pharmacy.
EXPERIENCE: None required.
ADDITIONAL REQUIREMENT: Due to insurance company requirements, incumbent must be 21 years of age or older, due to incremental use of a company car.
LICENSURE OR CERTIFICATION: Pharmacy Intern Licensure in the State of Arizona. Valid Arizona driver's license in good standing with the Arizona Department of Transportation. Heartsaver CPR (HSCPR or HSFACPR) or Basic Life Support (BLS) certification required. .
KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of drugs, possible side effects or interactions with other drugs, and how to compound medication in accordance with professional standards and practices.
  • Knowledge of patient interview techniques and documentation of medication histories and patient allergies
  • Basic computer skills necessary to obtain clinical information from the electronic medical record or online medical references
  • Skill in following physicians' orders and prescribed medications and relaying any procedures and concerns to the patient.
  • Skill in responding to patients' questions or concerns.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write and type clinical progress notes
  • Ability to effectively present information and respond to inquiries or complaints from employees, patients and/or their representatives, and the general public.
  • Ability to apply advanced mathematical concepts such as exponents or logarithms.
  • Ability to apply mathematical operations to such tasks as frequency distribution, single or multiple regressions, determination of test reliability and validity, correlation techniques, sampling theory and factor analysis.
  • Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems.
  • Ability to deal with nonverbal symbolism (formulas, scientific equations, graphs, etc.).
  • Ability to deal with a variety of abstract and concrete variables.
