As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Providing virtual and telephonic educational support tailored to the specific needs of your assigned patients, helping them navigate their mental health journey.
• Serving as the principal point of contact for patients and other healthcare professionals, ensuring effective coordination of care throughout the treatment pathway.
• Assessing patients' support needs and preferences to develop personalized care plans aimed at fostering successful treatment outcomes.
• Instructing patients on program policies and procedures while meticulously documenting all interactions and history in clinical charts to maintain accurate records.
• Tracking and documenting patient progress regularly, ensuring that all necessary information regarding patient status is available for the health team’s review.

In this role, you will work with a diverse patient population, including individuals experiencing anxiety, depression, trauma, and various socio-economic challenges that impact their mental health. Your work will help broaden access to mental health services, ultimately fostering supportive relationships that empower clients to take positive strides in their therapy journey.

To be considered for this role, candidates must possess the following qualifications:

• A Master’s degree in Social Work, Counseling, or a related field is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California is a must.
• A minimum of three years of experience working in counseling or a related capacity, demonstrating a solid background in mental health practices.
• Exceptional communication skills with the ability to communicate clearly and effectively in both written and verbal formats.
• A capacity to thrive in a fast-paced, competitive environment while maintaining a client-centric approach.
• Intermediate to advanced proficiency in computer skills, specifically in Microsoft Office Suite, particularly Word, Outlook, and Excel.

While the above qualifications are essential, candidates with prior experience in telehealth settings, familiarity with electronic health record (EHR) systems, or specialized training in cognitive-behavioral therapy will be viewed favorably.
