Description:Registered Nurse โ€“ Pulmonary Rehabilitation CoordinatorHelp Patients Breathe Better and Live Healthier

Indiana Internal Medicine Consultants is seeking a compassionate Registered Nurse to coordinate our outpatient Pulmonary Rehabilitation Program. This rewarding role combines patient care, education, chronic disease management, and care coordination to help patients improve their respiratory health and quality of life while reducing hospital readmissions.


Key Responsibilities
  • Coordinate all aspects of the outpatient pulmonary rehabilitation program.
  • Schedule and assess patients referred to pulmonary rehabilitation.
  • Monitor patient vital signs, oxygen saturation, and overall condition during rehabilitation sessions.
  • Educate patients on COPD, medications, treatments, lifestyle modifications, and disease management.
  • Provide transitional care support and follow-up for patients with qualifying pulmonary diagnoses.
  • Document patient progress and education in the electronic medical record.
  • Assist with pulmonary rehabilitation maintenance sessions.
  • Support Reclast infusion services, including patient education, IV starts, and medication administration.
  • Maintain AACVPR program certification requirements, outcome tracking, and staff training coordination.

Requirements:

QUALIFICATIONS:

  • Associates degree in Nursing.
  • Registered Nurse
  • Experience in pulmonary care, rehabilitation, chronic disease management, or ambulatory care preferred.
  • Minimum three years of nursing experience
  • Minimum two years training and educating clinic staff.
