Job Summary The Center for Advanced Practice (CAP) and Duke Health Integrated Practice (DHIP) employs a Team Leader model to advance its vision of a cohesive, goal-driven center that promotes excellence in advanced clinical practice across the healthcare continuum. The Team Leader provides leadership and support to Advanced Practice Providers (APPs), fostering clinical excellence, quality outcomes, effective communication, professional development, and collaboration within the team and across service lines while also participating in strategic planning and decision-making to align with CAP, DHIP and DUHS goals.
In addition to team leadership responsibilities, the Advanced Practice Provider (APP) serves as a key member of the Duke Specialty Infusion Center (DSIC). Working collaboratively with medical, nursing, and pharmacy colleagues—and under the supervision of the DSIC Medical Directors—the APP delivers direct patient care within the infusion center. Responsibilities include patient triage, performing physical examinations, collecting and documenting clinical data, conducting diagnostic and therapeutic procedures, ordering and coordinating laboratory studies and specialty consultations, serving as the first responder for patient needs, prescribing appropriate interventions and medications, and ensuring seamless coordination of care. The APP supports excellence in patient care, education, research, and contributes to the overall leadership and advancement to the organization and the APP profession.
Job Duties and Responsibilities Utilize assessment, diagnostic and clinical skills and knowledge in order to provide clinical care to patients during their admission. Perform necessary diagnostic and therapeutic procedures (within the scope of practice of the supervising physician). Conduct and obtain patient histories, perform physicals examination, monitor patient progress, order appropriate medications, laboratory studies and diagnostic procedures. Prescriptive practice with the exception of chemotherapy and IMID therapy: per Duke policy Document the patient's course of treatment at the direction of the supervising physician: History/ Physical/Admissions Problem List and Plan of Care Daily Progress Note Procedure Note Interval Notes Discharge Summary Accept scheduled and unscheduled inpatients Evaluate potential admission in the ER to facilitate patient care. Facilitate daily rounds ion the unit. Collaborate with other program and non-program services, physicians and nursing staffs in order to provide a longitudinal continuity of care between the inpatient and outpatient setting, and through follow-up Participate in meeting the educational requirements of students, medical residents, fellows and other members of the health care team Collaborate with members of the health care team in helping to assure compliance with research related protocols. Collaborate with the nurse manager in the identification of problems and their resolution in order to assure the providing of safe, but cost effective patient care Actively participate in program related committees and/or projects Provide a summary report on the status of inpatients to the attending and medical resident/fellow on call prior to leaving each evening Assure proper charges have been posted and appropriate documentation provided Work with team on quality for the unit. Team Lead Responsibilities Complete required leadership training, such as Guide to Managing at Duke and other training as requested. Be familiar with DUHS, DHIP and CAP policies and guidelines and utilize resources appropriately. Meet individually on a monthly basis with the CAP/DHIP director for coaching/mentoring. Review, plan, and discuss team successes, challenges, and opportunities. Participate as a member of the Leadership Team in overall decision-making, problem-solving, strategic planning, and implementing DUHS and CAP and DHIP goals/initiatives. Collaborate and plan future directions by attending regularly scheduled Leadership meetings and retreats. Respond to other leadership directives in a timely manner. Serve as a role model and mentor for team members, demonstrating professionalism in conduct, speech, appearance, work habits, customer service, communication style, critical thinking skills, resource utilization, and problem resolution. Establish strong, positive, professional relationships with medical, nursing, administrative, and community leaders connected with the clinical service unit (CSU). Seek out and participate in learning opportunities related to leadership development. Promote the professional development of healthcare team members. Successfully align their team to fulfill CAP, DHIP, CSU, and DUHS's mission, vision, goals, objectives, and strategies. Ensure adequate staffing to provide appropriate patient care. Provide oversight for schedule guidelines. Interview new APP applicants. Oversee with the Director, the new employee selection process and coordinate clinical onboarding/orientation. Ensure clinical competencies of team members are maintained. Participate with each team member in the professional performance review (Coaching Conversations) including midyear reviews and annual evaluations. Perform employee rounding
Job Eligibility Requirements Education NP: Graduate of accredited Family, Adult, or Acute Care Nurse Practitioner program PA: Graduate of ARC accredited Master's Degree or Doctoral Degree Physician Assistant Program Certification NP: FNP, AGPCNP or AGACNP PA: PA-C Licensure NP: Must have unrestricted RN licensure issued by or recognized by the NCBON with authorization to practice as a NP PA: Must have unrestricted license issued by NCMB with authorization to practice as PA Both: Federal and State DEA independent license with unrestricted prescriptive authority Experience APP experience is preferred, ideally in a comparable practice setting and with proven Leadership skills. Physical and mental requirements Able to clearly articulate scope of practice, and practices within those guidelines as a Nurse Practitioner / Physician Assistant in the state of North Carolina and within the Duke University Health System. Able to independently seek out resources and work collaboratively to solve complex problems Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others. Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up. Able to use fine motor skills Able to record activities, document assessments & interventions; prepare reports and presentations; Able to use computer and learn new software programs Able to provide leadership in clinical area of expertise and in meeting organizational goals Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds Able to remain focused and organized Able to work collaboratively with all levels of personnel Job Hours Primary hours will be 40 hours per week with rotating Monday through Friday, 12-hour day shift, Saturday and Sunday 10 hour shifts with hours flexible to meet the needs of the patients.
