Assistant Medical Director – Primary Care

At Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC), we’re dedicated to offering accessible, high-quality healthcare to New Jersey residents. Our comprehensive services span preventive care, primary care, wellness support, mental health counseling, prenatal care, and more, all in a compassionate, community-focused environment.


Position Summary:

We’re seeking an Assistant Medical Director who’s energized by purpose and committed to expanding access to outstanding primary care. In this full‑time Keyport‑based role, you’ll deliver hands‑on patient care while shaping clinical quality, supporting providers, and strengthening care delivery across our CHC network.


Principal Responsibilities:

  • Deliver high‑quality primary care (approx. 80% clinical / 20% administrative).

  • Maintain an active patient panel and model evidence‑based, culturally responsive care.

  • Support clinical operations as an EHR Superuser, including onboarding and workflow optimization.

  • Lead and participate in quality improvement, chart audits, peer review, and QI/QA activities.

  • Facilitate efficient provider and clinical team meetings, including agenda planning and follow‑up.

  • Collaborate with Medical Director and Finance to monitor and achieve productivity goals.

  • Support HRSA Operational Site Visit preparation and ensure compliance with HRSA, QRM, and FTCA standards.

  • Foster strong communication and positive, collaborative relationships across all teams.


Competencies & Minimum Qualifications:

  • Graduate of an accredited medical school and residency program

  • Board certification in Family Medicine or Internal Medicine

  • Active New Jersey medical license or eligibility for licensure

  • Ability to obtain and maintain federal and state CDS registration

  • 3–5+ years of clinical practice experience, including ambulatory care; FQHC experience preferred

  • Strong understanding of population health, quality metrics, and clinical analytics

  • Excellent verbal, written, and interpersonal communication skills

  • Bilingual English–Spanish preferred

  • Proficiency with computers, data analysis, and Microsoft Office (including advanced Excel); nursing informatics background preferred

  • Valid NJ driver’s license and access to a personal vehicle

Working Conditions/Physical Demand: Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing, walking, lifting 40 pounds, pushing, reaching, pulling; working with thermometer, sphygmomanometer, stethoscope, otoscope, ophthalmoscope, needles, syringes, EKG machine, venipuncture equipment, other diagnostic equipment, spirometry treatment, culture mediums, telephone, fax, photocopier, computer, printer.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $200,000.00  to USD $$296,000.00. When determining an offer, VNACJ CHC considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
