Job Purpose & Position Overview:
The purpose of this position is to coordinate, implement, and evaluate community-based chronic disease prevention and management initiatives that improve the overall health and wellness of community members. The Chronic Disease Prevention, Wellness, and Screening Coordinator will provide leadership for chronic disease education, lifestyle coaching, fitness and wellness promotion, community health screenings, and care coordination activities designed to reduce risk factors and improve health outcomes.
This position serves as a bridge between community members, healthcare providers, public health programs, and community resources to increase access to preventive services, support healthy lifestyle changes, and promote early detection of chronic diseases. The Coordinator will lead efforts related to diabetes prevention and management, cardiovascular health, obesity prevention, nutrition, physical activity, tobacco cessation, healthy aging, and other chronic disease priorities. The position will also oversee community health screenings, referral processes, wellness activities, and population health initiatives that align with the Winnebago Public Health Department's strategic goals and community health improvement priorities.
Specific Job Duties/Responsibilities:

• Coordinate chronic disease prevention and management activities targeting diabetes, hypertension, cardiovascular disease, obesity, and other chronic conditions.
• Develop and implement evidence-based programs that support disease prevention, self-management, and healthy behavior change.
• Monitor participant progress and assist with care coordination, referrals, and follow-up services.
• Support community members in achieving individualized health and wellness goals.
• Serve as a Lifestyle Coach for evidence-based programs such as the National Diabetes Prevention Program (NDPP) and other chronic disease prevention initiatives.
• Provide individual and group coaching related to nutrition, physical activity, stress management, sleep health, tobacco cessation, and healthy lifestyle behaviors.
• Facilitate wellness classes, workshops, support groups, and educational events.
• Promote behavior change strategies utilizing motivational interviewing and culturally responsive approaches.
• Coordinate community fitness and physical activity initiatives.
• Develop and oversee wellness challenges, walking programs, fitness classes, and community exercise opportunities.
• Collaborate with fitness staff, healthcare providers, schools, and community organizations to increase participation in physical activity programs.
• Assist with development of policies and environmental strategies that support healthy living.
• Plan, coordinate, and implement community health screening events.
• Conduct and oversee screenings including blood pressure, blood glucose, A1C, cholesterol, BMI, weight management, and other preventive health measures.
• Ensure screening results are appropriately documented and communicated.
• Coordinate referrals and follow-up for individuals identified as at-risk during screenings.
• Maintain screening data and assist with evaluation of program outcomes.
• Analyze community health data and identify trends, priorities, and opportunities for intervention.
• Participate in development of action plans and implementation strategies that address chronic disease risk factors.
• Develop and maintain collaborative relationships with healthcare providers, community organizations, schools, employers, and tribal programs.
• Coordinate outreach activities designed to increase participation in preventive health services.
• Promote awareness of available resources and services that support healthy living.
• Represent the program at community meetings, health fairs, conferences, and wellness events.
• Collect, maintain, and report program data and performance measures.
• Assist with grant management, work plans, budgets, and required reporting activities.
• Monitor progress toward grant objectives and performance targets.
• Prepare reports, presentations, and program summaries for leadership and funding agencies.

Qualifications/Skills:

• High School Diploma or GED required and a Associates degree in a health-related profession or higher preferred
• Must have advanced knowledge and a minimum of two (2) years of experience in field/public health.
• Experience coordinating community health screenings and wellness programs.
• Experience working with diabetes, chronic disease prevention, population health, or public health programs.
• Knowledge of motivational interviewing, health coaching, and behavior change techniques.
• Experience working with Indigenous communities and culturally responsive health promotion approaches preferred.
• Knowledge of Diabetes Practice Guidelines and Case Management Techniques.
• Knowledge of principles of diabetes education and culturally appropriate delivery models.
• Must have a valid driver's license.
• Must pass required background checks.

Physical Requirements:

• Normal to light office work which includes the ability to lift up to 10 lbs
• Regularly required to walk, stand, and sit for extended periods of time and use hands to operate computer keyboard, phone, and various office equipment.

Language Requirements:

• Must possess the ability to read, write, and speak the English language fluently.
