Physical Therapist

Deliver hospital-level & home health care—from the comfort of home

Activa Home Health is seeking experienced and clinically strong Physical Therapists (PTs) to join our team supporting both our Medicare home health patients and when needed our Hospital at Home cases. We are hiring for full-time and per-diem roles across Indian River, St. Lucie, and Martin Counties.

This role is ideal for therapists who thrive in acute care, home health, and high-acuity environments, and are passionate about restoring mobility, improving safety, and supporting patients' independence at home.

Position Summary

The Physical Therapist provides skilled, patient-centered therapy services for both acute-level (Hospital at Home) and Medicare home health patients, focusing on mobility, fall prevention, functional independence, and successful recovery outcomes.

Key Responsibilities

  • Perform comprehensive PT evaluations, including: strength, balance, endurance, and functional mobility; gait analysis and fall risk assessment; transfers, bed mobility, and stair negotiation; activity tolerance and cardiopulmonary response
  • Develop and implement individualized, evidence-based treatment plans
  • Address functional decline related to acute illness, injury, or recent hospitalization
  • Provide gait training, balance interventions, and mobility progression
  • Educate patients and caregivers on safe mobility and transfer techniques; fall prevention and home safety; use of assistive devices and equipment
  • Monitor patient progress and communicate changes with the interdisciplinary team
  • Complete accurate and timely documentation, including OASIS assessments (preferred)
  • Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce readmissions

Required Qualifications

  • Active Florida Physical Therapist (PT) license
  • Degree from an accredited Physical Therapy program (DPT preferred)
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Strong clinical reasoning, communication, and time-management skills

Preferred Experience

  • Medicare home health experience
  • OASIS experience preferred
  • Acute care or hospital-based physical therapy
  • Hospital at Home, home health, or transitional care experience
  • Experience treating medically complex patients
  • Familiarity with EMR systems
