Nurse Practitioner – Mobile Primary Care Provider (Medicare FFS)

Virtue Mobile Care is a physician-led organization dedicated to providing relationship-centered, proactive primary care in the home for Medicare beneficiaries. As we expand into the Redding market, we are seeking a compassionate and independent Nurse Practitioner to help deliver high-quality, patient-focused care directly where patients live.

Position Summary

The Nurse Practitioner (NP) will provide in-home medical care throughout an assigned service area. Responsibilities include routine follow-up visits, chronic disease management, acute care visits, preventative care, and patient education. This role supports patients and works collaboratively with their primary care providers and other healthcare professionals to improve health outcomes and reduce unnecessary hospitalizations.

Preferred Qualifications:

• Fluency in Vietnamese, Mandarin and Cantonese to serve diverse patient populations

Key Responsibilities

Patient Assessment

• Conduct comprehensive physical examinations and health assessments.
• Evaluate and monitor patient conditions.
• Review and interpret diagnostic studies, including laboratory results, imaging reports, and other clinical data.

Diagnosis and Treatment

• Diagnose and manage acute and chronic medical conditions.
• Develop treatment plans and prescribe medications and therapies as appropriate.
• Provide evidence-based care in accordance with regulatory and organizational standards.

Patient and Family Education

• Educate patients and caregivers regarding diagnoses, treatment plans, medications, and preventive health measures.
• Promote patient engagement and self-management strategies to improve overall wellness.

Care Coordination

• Collaborate with physicians, case managers, specialists, and other healthcare team members.
• Participate in developing and implementing comprehensive care plans.

Documentation and Administrative Duties

• Document patient encounters accurately and timely within the electronic health record (EHR).
• Complete required clinical and administrative documentation.
• Maintain compliance with Medicare and organizational requirements.

Professional Development

• Participate in staff meetings, educational programs, and ongoing training opportunities.
• Maintain clinical competency by following current best practices and quality standards.

Additional Skills

• Strong organizational, problem-solving, and time-management abilities.
• Ability to work independently in varied and sometimes unpredictable home-based care settings.
• Proficiency with electronic health records (EHRs) and telehealth technology.

Work Environment

• Travel throughout the community to patients' homes, assisted living facilities, community centers, and other designated locations.
• Independent field-based work with minimal direct supervision.
• Requires flexibility, effective scheduling, and strong clinical judgment.

Required Qualifications (California)

• Active California Nurse Practitioner License.
• California NP Furnishing Number (or ability to obtain).
• DEA Registration (or ability to obtain prior to providing patient care).
• California Schedule II Controlled Substance Continuing Education, if prescribing controlled substances.
• Valid California Driver's License and reliable transportation.
• Compliance with California AB 890 requirements, including practice under Standardized Procedures or independent practice eligibility upon completion of the 4,600-hour transition requirement.

Schedule

• Primarily Monday through Friday, daytime hours.
• Occasional weekend availability based on patient care needs.
• No routine night call or 24/7 on-call coverage requirements.

Compensation & Benefits

Revenue Share Model

• Compensation is based on a revenue-share structure.
• Nurse Practitioners receive 60% of Medicare Part B Fee-for-Service collections generated from services provided.

• Credentialing, billing, scheduling support, and administrative services provided by Virtue Mobile Care.

Work Location

On the Road / In-Person

Patients are seen in their homes and community-based settings throughout the assigned service area.
