We’re hiring a Registered Nurse to support adults with disabilities by coordinating healthcare services, monitoring medical needs, and helping ensure individuals receive the highest quality of care. In this role, you’ll work closely with individuals, healthcare providers, guardians, and support staff to promote health, wellness, and independence.

What You’ll Do:

Care Coordination & Advocacy

  • Manage a caseload of individuals and coordinate medical appointments, treatments, and follow-up care
  • Participate in care planning meetings with healthcare providers, guardians, and support teams
  • Advocate for individuals to ensure they receive appropriate medical services and supports
  • Evaluate the need for health-related equipment and assist with implementation as needed

Health Monitoring & Support

  • Monitor medical treatments and health-related services to ensure quality care
  • Assess ongoing healthcare needs and communicate recommendations to the support team
  • Collaborate with Direct Support Professionals to promote best practices in health and wellness

Training & Oversight

  • Monitor treatment practices and provide guidance to DSPs regarding medical supports and procedures
  • Assist in maintaining quality healthcare services for individuals receiving support
  • Support compliance with healthcare-related policies, procedures, and documentation requirements

What You Bring:

  • Current Registered Nurse license required
  • Minimum of 2 years of professional nursing experience in public health or a related setting
  • Minimum of 2 years of experience working with individuals with disabilities preferred
  • Strong communication, organizational, and problem-solving skills
  • Compassionate, patient-centered approach to care
  • Ability to work collaboratively with individuals, families, healthcare providers, and support staff
