Job description

Opportunity Details

  • This position is a hybrid role, with 0.5 FTE dedicated to providing primary care coverage and 0.5 FTE inbasket management.

  • The in-office primary care responsibilities include delivering acute care services, conducting follow-up appointments for hospitalized patients, and ensuring continuity of care until patients are able to see their primary care providers. Your office location will be at the Windsor Primary Care building in Urbana, IL.

  • The inbox management duties involve overseeing provider inboxes for multiple primary care clinics when providers are unavailable due to leave, vacation, or continuing medical education.

  • This is an innovative and unique primary care role designed to improve patient access, more efficiently manage primary care workloads and reduce primary care provider burnout.

  • Schedule will be M-F 8a-5p in person at the Windsor location. Your schedule will be determined based on primary care access needs.

  • You would join a team of dedicated and collaborative team of providers and experienced support staff in our clinic. You will also be able to consult with specialists across the organization to effectively care for patients.

  • Rotate in a shared-on call group with the Windsor Primary Care providers via phone.
