As a Registered Nurse, you'll bring expertise, confidence, and decisiveness to every interaction. You'll provide direct and indirect nursing care to participants, managing complex clinical situations and ensuring care is delivered safely and effectively.

Schedule:
First shift, full-time hours. Approximately 20% of your work will involve home visits. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:
  • Provide direct and indirect nursing care to participants following the Care Plan.
  • Collect and analyze patient data, set priorities, and identify problems needing follow-up.
  • Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.
  • Provide education, guidance, and discharge instructions to participants and families.
  • Respond independently to emergencies and implement advanced care as needed.
  • Document care, observations, and participant outcomes accurately in the electronic medical record.
  • Communicate participant changes and updates clearly to the care team.
  • Participate in Quality Improvement initiatives and clinical process enhancements.
  • Travel to participant homes or other sites as needed to provide care.

Education and Experience:
  • Must be a graduate of an accredited nursing school and have a current RN license within practicing state.

  • Experience working with the frail or elderly population is a plus - but not required. We'll provide hands-on training and mentoring to help you grow!

Requirements:
  • Requires valid State driver's license with a clean driving record.
  • Successful completion of background check, physical, drug screening, TB Screening

Physical Requirements:
  • Must be able to move intermittently throughout the workday.
  • Must function independently, maintain confidentiality, have flexibility, personal integrity, and the ability to work effectively with personnel, regional and corporate consultants, vendors, patients/participants and support agencies.
  • Is subject to lifting, carrying, and supporting participants.
  • May include occasional overnight travel for in-service training programs, temporary coverage for another Center, etc.
  • Must be able to drive a motor vehicle.
