Find your calling at Mercy!Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.Position Details:Primary Care RN or LPN - Mercy Clinic Internal Medicine - Sullivan

Schedule: Monday - Friday

No weekends or holidays!

**Must complete a minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.**

LPN Qualifications:

Education: Diploma in Practical Nursing

Licensure: Active LPN license

Experience: 2-3 years of experience strongly preferred

RN Qualifications:

Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor's degree

Licensure: Active RN license

Experience: 2-3 years of experience strongly preferred

Highly Preferred Skills:

Proficiency with Epic EMR system.

Timely and thorough patient triage skills.

Computer competency (most of this position will be computer work).

Excellent time management and prioritization.

Adaptability in a new and changing environment.
