Pay Rate: RN: $40.00-$50.00 per hour (based on experience)
Employment Type: Full-Time and Part-Time
RN/ Registered Nurse
We are seeking compassionate and dedicated Registered Nurses (RNs) to provide skilled nursing care to clients in their homes. The RN will be responsible for conducting comprehensive assessments, developing and managing care plans, coordinating services with healthcare providers, and educating clients and their families to promote optimal health outcomes.
Key Responsibilities
Clinical Care & Assessments
Conduct initial and ongoing comprehensive client assessments.
Evaluate medical, psychosocial, emotional, cognitive, developmental, and safety needs.
Develop, implement, and revise individualized care plans.
Monitor client conditions and evaluate treatment effectiveness.
Provide specialized nursing services and interventions as ordered by physicians.
Care Coordination
Communicate and collaborate with physicians, psychologists, interdisciplinary teams, hospitals, and community resources.
Coordinate services and act as a case manager when appropriate.
Participate in case conferences and discharge planning.
Patient & Family Education
Educate clients and caregivers on disease processes, medications, safety precautions, self-care techniques, and treatment plans.
Train clients and caregivers on home healthcare procedures and equipment use.
Documentation & Compliance
Maintain accurate and timely clinical documentation.
Report changes in client condition to physicians and clinical leadership.
Ensure compliance with agency policies, state and federal regulations, infection control standards, and quality assurance programs.
Additional Responsibilities
Administer medications, injections, IV therapies, lab draws, and specimen collections as required.
Supervise LPNs and paraprofessional staff.
Participate in ongoing education, training, and professional development.
Qualifications & Requirements
Must be a graduate of an accredited School of Nursing.
Must be a Registered Nurse licensed by the state of Maryland.
A minimum of two (2) years of experience as a professional nurse in a Home Health Agency is preferred
Must have a criminal background check.
Must have current CPR certification.
Strong clinical assessment, care planning, and critical thinking skills.
Excellent communication, documentation, and interpersonal skills.
Ability to work independently and effectively coordinate care with physicians, caregivers, and interdisciplinary teams.
Reliable transportation and valid driver's license may be required for home visits.
Ability to maintain confidentiality and comply with all applicable federal, state, and agency regulations.
