Location: East Honolulu, Oahu, Hawaii

Are you a compassionate Registered Nurse looking for flexibility, autonomy, and the opportunity to make a meaningful impact in your community?

Always Best Care Home Health is seeking a dedicated Registered Nurse (RN) to provide skilled nursing services to patients throughout East Honolulu. This per diem position offers the flexibility to build your own schedule while delivering one-on-one care in patients' homes.

Compensation

Competitive visit-based compensation, including:

โ€ข Routine Skilled Nursing Visits: $80 per visit

โ€ข Start of Care (SOC) Visits: $160 per visit

โ€ข Initial Evaluations, Recertifications, Re-Evaluations, Resumption of Care, and Discharge Visits: $120 per visit

โ€ข Orientation and Onboarding: $50 per hour

Additional compensation may apply for visits that exceed standard visit durations.


Special Consideration

We are especially interested in Registered Nurses with experience completing OASIS Start of Care (SOC) assessments.

Additional opportunities may be available for clinicians experienced in SOC admissions, recertifications, OASIS documentation, and other Medicare home health assessments.

While home health and SOC experience are preferred, we welcome applications from all qualified RNs who are passionate about providing exceptional patient care in the home setting.

About the Role

As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while working closely with physicians, caregivers, and the interdisciplinary care team to support positive health outcomes.


Responsibilities

โ€ข Conduct skilled nursing visits in patients' homes

โ€ข Complete comprehensive assessments and reassessments

โ€ข Develop and update individualized care plans

โ€ข Provide patient and family education

โ€ข Perform wound care, medication management, home infusions, disease management and other treatments.

โ€ข Coordinate care with physicians and interdisciplinary team members

โ€ข Monitor patient progress and communicate changes in condition

โ€ข Complete accurate and timely clinical documentation

โ€ข Ensure compliance with Medicare home health standards and agency policies


Qualifications

โ€ข Active Hawaii Registered Nurse (RN) license

โ€ข Minimum one year of clinical nursing experience

โ€ข Home health experience preferred but not required

โ€ข Experience with OASIS SOC assessment preferred

โ€ข Strong assessment, clinical judgment, and documentation skills

โ€ข Ability to work independently and manage a patient caseload

โ€ข Reliable transportation and willingness to travel within the East Honolulu service area

โ€ข Strong communication and care coordination skills
