In this role, you'll support patients with complex conditions by coordinating care, connecting resources, and empowering individuals to take control of their health.
Compensation
We offer competitive pay based on education, experience, and skills:
  • LPN: Starting at $56,160/year, commensurate with experience
  • RN (Associate Degree): Starting at $76,968.35/year, commensurate with experience
  • RN (Bachelor's Degree): Starting at $78,200/year, commensurate with experience
Work Schedule
  • Monday-Friday
  • 8-hour shifts
  • Scheduled during clinic operating hours: 7:00 AM-6:00 PM
What You'll Do
Care Coordination & Case Management
  • Conduct comprehensive assessments of patient needs (medical, social, behavioral, financial)
  • Develop and implement individualized, cost-effective care plans
  • Collaborate with providers, behavioral health teams, and specialists
  • Coordinate referrals and community resources
  • Monitor services to ensure high-quality, efficient care delivery
