Accredited by The Joint Commission (TJC), SCH is made up of South County Hospital, South County Home Health, and South County Medical Group. Our new physician will see patients within a patient-centered medical home model offering strong collaboration with a pharmacist and nurse case manager to support high-risk patient populations. The Center allows our physicians to coordinate care with specialists within the group. Practice patients receive breathing, intravenous antibiotics, and other treatments, suturing and other procedures, and after-hours care by the adjacent Express Care, enabling seamless transitions of care. On-site diagnostic services include x-ray, ultrasound, and 3D tomosynthesis, as well as a lab draw station. On-site Ob/Gyn and dermatology providers and outpatient physical therapy round out the many oppor­tunities for clinical collaboration and create a one-stop care opportunity for our patients.

The candidate must be board-certified or board-eligible in Internal or Family Medicine. He or she will be part of the South County Medical Group (SCMG), a comprehensive network of providers that includes a variety of specialists, as part of South County Health.
