Company Summary: Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women, and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.


Due to federal contracts the following items are prohibited: Medical/Recreational marijuana, current parole, or probation.


Job Summary: The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring clients' health through the nursing process; collaborating with interdisciplinary team members; providing physical health support and education to clients. Assessment of the physical needs of the client and implementation of a care plan through preventative and therapeutic services in accordance with all state, federal, and licensing standards and guidelines.


Supervisory Responsibilities:

  • None

Essential Functions:

  • Conducts an individualized and ongoing nursing assessment on each patient as directed by the provider or management.
  • Assists with initial health assessment of client upon admission as needed.
  • Executes the plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team.
  • Performs appropriate treatments as ordered by provider in an accurate and timely manner.
  • Performs therapeutic nursing interventions as established by individualized plan of care for the patient.
  • Delivers care with appropriate age and cultural competence to specific patient populations according to individualized needs.
  • Travels to each Crossroads facility and/or community locations to administer, but not limited to, medications, venipuncture, suboxone inductions, immunizations and injections, wound care, and EKGs.
  • Coordinates prescription orders, pick-ups and deliveries from community and specialty pharmacies and initiate necessary paperwork when needed.
  • Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
  • Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
  • Maintains confidentiality in matters related to patients.
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient's and family's diversity, preserving their autonomy, dignity, and rights.
  • Reports patient condition to appropriate personnel and/or facility during each shift.
  • Responsible for managing their own schedule and tasks within the EMR system.
  • Orient the clients to the Clinic system and available services.
  • Act as an advocate in directing clients to available Clinic and community resources.
  • Educate clients regarding condition, treatment, and possible outcomes.
  • Provide health care services for identified prevention and health promotion needs.
  • Maintain accurate medical records using EMR and proper forms in a timely manner.
  • Consistently use supplies and equipment in a cost-effective manner.
  • Performs other duties, as required.

Qualifications:

  • Current CPR/First Aid/Narcan training, BLS preferred.
  • Current and annual negative TB Test.
  • Current and valid Fingerprint Clearance Card.
  • Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
  • Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.


Knowledge, Skills, and Abilities:

  • Excellent leadership, communication, problem solving and organizational skills.
  • Ability to work independently and as a member of a team.
  • Strong knowledge of EHR and EMR records systems.
  • Ability to adhere to all in-house and legal regulations.

Education & Experience:

  • Graduate of an accredited school of nursing.
  • Valid, state-issued Nurse license in good standing.
  • 2+ years' clinical experience in a clinical care setting.
  • Experience with clinical risk management and safety protocols.


Work Environment:

  • Fast paced behavioral health setting.
  • Primarily indoors, however, some outdoor
    work may be requested.


Physical Demands:

  • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.

  • May include travel between sites and other locations.
  • Local and statewide travel when requested or required.
