Job Description

Join our team as a Temporary Social Worker. In this role, you will be responsible for providing social work services to the residents and family members of Continuing Care.

Compensation: $35-$36 an hour (depending on experience)

How you will make an impact

  • Completing admission assessments, MDS, and other required documentation in accordance with State and Federal regulations
  • Educating residents/families of the importance of advance directives and facilitating completion, signature, and tracking of documents
  • Coordinating the Social Work function from admission to discharge to include room changes and transition planning
  • Providing psychosocial support to residents and families to ensure a successful transition to Continuing Care
  • Participating in care/service plan meetings with family members
  • Providing bereavement support to residents, family members, and staff

What you will need

  • Social Work degree (BSW or MSW) required, or minimum education requirements as per state licensure.
  • Minimum of 3 years' experience in working with the older adult population in post-acute/ long term care preferred.
  • Knowledge of geriatric population and dementia.
