Key Responsibilities
  • Conduct comprehensive assessments of patients' mental health needs, ensuring a thorough understanding of their psychological, social, and emotional conditions.
  • Develop and implement personalized treatment plans using evidence-based interventions tailored to the unique circumstances of each patient, particularly focusing on pain management and emotional support during end-of-life transitions.
  • Utilize specialized therapy techniques such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT) to guide patients and families through their journeys, addressing issues such as grief, anxiety, and depression.
  • Maintain accurate and up-to-date clinical records in strict accordance with confidentiality standards and compliance regulations, ensuring that documentation reflects the care provided.
  • Collaborate closely with a multidisciplinary treatment team, participating in regular case reviews to ensure a holistic approach to patient care and continuity in treatment.
  • Educate patients and their families about mental health resources, coping strategies, and support systems available to them during the hospice process.

Patient Population / Client Focus

This role primarily focuses on serving diverse patients facing terminal illnesses, including their families. Qualifications
  • A Master’s degree in Social Work from an accredited institution is essential.
  • Possession of a valid LCSW license in the state of Illinois.

  • Proficiency in evidence-based psychotherapy modalities, particularly solutions-focused interventions.

Compensation and Benefits
