Job description

Responsibilities

The role of the Community Integrated support Specialist is to provide support and education to people with mental illness while networking to community based supports and participating in client treatment plans

Responsibilities include:

developing client centered treatment plans with measurable goals and objectives, meeting with clients as clinically necessary, monitoring progress of clients receiving waiver support services along with other skill development services, understanding and abiding by DHS and Medica guidelines as well as client confidentiality requirements and standards

Essential duties of the ICS support staff are coordinating services and community supports, working with representatives from other agencies as appropriate to better support clients, flow chart maintenance, timely and accurate completion of all required paperwork, attending all required meetings, trainings and supervisions as required

Driving will be required in this position

Hours will vary based on client and program needs
