About iCare

iCare Health Care Services is a non-medical home care agency serving Milwaukee, Waukesha, and Ozaukee Counties. We help seniors and adults with disabilities remain safely and independently at home through companion care, personal care, and support services.

The Role

iCare is seeking a Wisconsin-licensed Registered Nurse to conduct in-home client care needs assessments and provide clinical oversight of our caregiver staff, in compliance with Wisconsin DHS Chapter 105 regulations. This is a flexible, part-time/PRN position well suited to an RN seeking supplemental hours outside a primary clinical role.

Key Responsibilities

● Conduct in-home Client Care Needs Assessments for new clients and reassessments per DHS 105.17

● Develop and periodically update individualized plans of care

● Provide clinical supervision and competency oversight of caregiving staff

● Review and sign off on caregiver training documentation

● Serve as a clinical resource for care coordination questions from the intake and operations team

● Maintain accurate, compliant clinical documentation

Qualifications

● Active, unrestricted Wisconsin RN license

● 1+ years of home health, home care, or community health nursing experience preferred

● Familiarity with Wisconsin DHS Chapter 105 non-medical home care regulations a plus

● Reliable transportation for in-home visits across the five-county service area

● Strong documentation and communication skills

Schedule & Location

Flexible, part-time/PRN schedule (approx. 8-15 hrs/week to start, scaling with caseload). Field-based, with visits across Milwaukee, Waukesha, and Ozaukee Counties.
