Job description


Role and Responsibilities

Responsible for developing and maintaining client relations and intake of direct referrals made to HCR.

Essential Functions

  • Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients.
  • Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).
  • Monitor Intake Team email.
  • Produce timely, accurate data entry.
  • Responsible for entering orthopedic prescreens into HCHB according to agency protocol.
  • Provide phone support to the department and triage of calls to Intake staff.
  • Provide excellent customer service to internal and external customers.
  • Participate in department and agency meetings.
  • Other duties as assigned.

Education Requirements

  • Graduate of an accredited program of practical nursing required along with valid and unencumbered LPN license issued by NYS dept of education.

Qualifications and Requirements

  • Minimum of 4 years experience in a clinical setting; medical office experience preferred.
  • Exceptional customer service and communication skills required.
  • Must be proficient and efficient working with a computer; experience with EMR, and MS Office products required.
  • Detail oriented.
  • Accustomed to working in a fast paced environment.

Work Environment

The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions.
