# Registered Nurse - Day HabHot JobOmni Way - Chelmsford, MA 01824## OverviewSalary Range$90,000.00 - $90,000.00 Salary/year## DescriptionSummaryResponsible for overseeing all health care services to Individuals served in the agency Day Habilitation, Community Based Day Services Program. Incumbent must comply with applicable regulatory requirements and standards in the performance of nursing duties.The Nursing Manager fulfills the role of Health Care Supervisor as defined in 130 CMR 419.425 of the Day Habilitation Program Regulations.Essential Job Functions* Responsible for all health care services provided to Individuals in the Day Habilitation Program* Provides direct nursing care, including the administration of medications and treatments prescribed by Individual’s PCP. Provides periodic audits of nursing documentation to ensure compliance with all regulatory requirements* Coordinates and collaborates with licensed clinicians regarding medical issues, decline in status and change in clinical needs.* Determines and assesses individuals appropriateness for services per Mass Health regulations.* Coordinates individuals return to program following hospitalizations and acute medical crises. Attends discharge planning meetings and coordinates with clinical team.* Serves as a member of the Professional Interdisciplinary Team for each Individual and provides continuing evaluation and review of each Individual’s needs and status.* Reviews and approves the Day Habilitation Support Plan (DHSP) for each Individual, ensuring that all goals and objectives are not medically contradicted. Makes recommendations for DHSP goals related to health care* Collaborates with individuals care providers around medical and nursing supports during program hours. Obtains reports and approval of treatments plans and reports changes in the condition of each Individual to PCP as needed.* Completes all required nursing documentation including monthly, quarterly and Annual Nursing Assessments for each Individual* Advises the Day Services Director, Assistant Directors, Program Managers and Day Habilitation staff regarding any medical problems that may hinder an Individual’s progress or may be aggravated by activities in the Program* Monitors each Individuals health status and documents those findings in the medical records as required. Makes recommendations regarding health and medical care as needed and monitors follow up.* Coordinates the medical and health related elements of the referral and intake process for incoming individuals in collaboration with the intake/referral review day habilitation team members* Ensures adequate nursing and medical supplies are on hand to provide quality nursing care.* Creates nursing protocols and care plans based on individuals specific support needs. Monitors delivery and implementation of all clinical and nursing services.* Reviews all incident reports and makes recommendations and/or follow up with appropriate parties.* Ensures medical information in Individual’s record is updated and accurate. Maintains medical data as required* Approves the Severity Profile documentation for each Individual as per policies and procedures* Oversees the handling of medical emergencies during the day. Provides CPR and First Aid.* Determines whether Individuals should continue to receive services at the Day Habilitation Program due to medical/health status* Provides crisis management; assures that human rights, safety and respect for Individuals are safeguarded in all interventions and interactions. Maintains documentation on emergency events* Provides training to program staff, as appropriate and applicable in medical/health-related topics, including but not limited to; Universal Precautions for New Employee Orientation and individual’s specialized medical protocols.* Works with the Assistant Clinical Director of Nursing in the development, revision and enforcement of all policies and procedures related to nursing services, including medication and treatment administration* Attends clinical meetings and quarterly review meetings* Fosters and maintains positive relationships with funding sources, residential programs and families. Manages family, Individual, staff, and community complaints quickly and proactively; informs supervisor of major and/or unresolved issues.* Serves as an active, contributing, problem-solving member of the agency team. Acts in a manner consistent with organizational values, program mission, and service delivery goals.
