The Care Coordinator will assist Swope Health with the implementation of the Population Health initiative at Swope Health. The Care Coordinator will be an active member of the care team and will work in collaboration with other team members to provide care coordination for clients with health care needs including a full array of physical health needs, behavioral health needs, long-term community care services, and family services for consumers.
CLINICAL COMPETENCY
• Ability to work independently, according to guidelines, protocols and/or standing orders and manage multiple responsibilities or emergency situations appropriately
QUALIFICATIONS
• Degree in or prior experience in a human services field or medical field such as psychology, education, nursing, sociology, behavioral science, social work, psychology, human development and family studies, counseling, child development, gerontology, rehabilitation counseling. Such as a Case worker, LPN, or Certified Medical assistant
• 3 to 5 years clinical or care management experience preferred
• Information technology software experience
• Basic Cardiovascular Life Support (BCLS) preferred
• Strong communication skills, customer service oriented, professional telephone etiquette and exhibits the ability to work effectively with people (staff, visitors and clients).
• Spanish Speaking preferred
