Job description

Title: Program Coordinator (one full-time position)
Project/Location: Clean Air for Neighborhoods and Nature (former CARE): Siskiyou County Project focused in Dunsmuir and Montague, CA
Salary: $28/hr. Status: Non-Exempt
The California Health Collaborative is recruiting one full-time Program Coordinator, for the Community Allies for Rural Equity. These positions are funded through 6/30/2028 under contract with the CA Tobacco Control Program.
This Project works to reduce tobacco related disparities and advance momentum for a tobacco free California. The PC will coordinate program activities, oversee general communications, gauge political temperature for local policy adoption, and support community norm change around tobacco use and control.
Under general supervision of the Project Director, the Program Coordinator supports the planning, implementation, and evaluation activities of the Clean Air for Neighborhoods and Nature project.
ESSENTIAL DUTIES
  1. Consults with the Project Director relative to planning, policy and program direction.
  2. Provides support in the development of functional program work plans, which are intended to direct program activities in the achievement of program goals and objectives.
  3. Participates in ongoing process and outcome evaluation.
  4. Builds partnerships on behalf of the program with community-based organizations, healthcare sites, Local Lead Agencies, community stakeholders, and targeted community colleges.
  5. Oversees community outreach and engagement activities in the Scope of work, including overseeing planning, training, and implementation activities: coordinating teleconferences; developing educational materials; and providing technical assistance and support services to community members and local tobacco control partners.
  6. Represents the California Health Collaborative at meetings with committees, community agencies, care providers and related entities, and facilitates inter-agency linkages, communication and collaboration.
  7. Develops and conducts presentations and trainings for groups and one-on-one meetings with key decision makers, community leaders and community partners.
QUALIFICATION REQUIREMENTS
  1. Applicants must live in Siskiyou County and have a good understanding of the community including familiarity with local history, organizations and agencies, and systems in place.
  2. Comfortable with public speaking.
  3. The ideal candidate will be highly organized and capable of managing, directing, and tracking activities in multiple cities at the same time. They will have a passion for community engagement and being an agent of change. They will be a strong advocate and leader that can withstand opposition and frustrations in working for change within a bureaucracy. Candidates are encouraged to include examples of work including data analysis, written reports, videos, infographics, and/or social media posts in addition to a resume and cover letter.
EDUCATION AND/OR EXPERIENCE
Bachelorโ€™s degree in Public Health, Social Work, Health Education or a related field is preferred.
A minimum two yearsโ€™ experience working in the geographical region of Siskiyou County with an understanding of the atmosphere and the communityโ€™s social norms. A combination of experience and education demonstrating an ability to fulfill the duties of the position will be considered.
A valid California Driverโ€™s License and proof of insurance is required.
Work Environment
Physical demands will include sitting, bending, lifting to 25 pounds, driving and walking.

Cross-Program Collaboration
Collaborate across CHC programs to increase awareness of CHC services, promote CHC-branded programs and standardized referral pathways, and consistently utilize CHCโ€™s online referral asset (including the CHW Navigation hub and related support programs) to strengthen access, coordination, and continuity of care.
External Awareness amp; Representation
Proactively promote CHC priorities, programs, and referral assets to all external stakeholdersโ€”including enrollees, partner organizations, and community entitiesโ€”to eliminate fragmentation, prevent missed referral opportunities, and ensure consistent awareness and utilization of CHC services.
Professional Standards and Virtual Meeting Expectations
Employees are expected to maintain a professional and business-appropriate appearance and conduct during all work-related interactions, including virtual meetings.
Camera Usage Requirement
The employee is required to have their camera turned on and remain visibly present during all company business meetings conducted through Zoom or other approved virtual meeting platforms unless otherwise authorized by management due to exceptional circumstances.
Employees participating in virtual meetings must present themselves in a professional manner consistent with workplace expectations, including maintaining appropriate attire, a professional appearance, and a distraction-free environment suitable for business communications.
