Job description

The YWCA Greater Harrisburg is seeking a passionate and dedicated full-time Permanent Supportive Housing Case Manager to join our team.
This position provides comprehensive case management and housing stabilization services to individuals and families experiencing homelessness through HUD-funded programs. This position assists clients in securing and maintaining safe, affordable housing by providing advocacy, crisis intervention, resource coordination, and individualized support. The Permanent Housing Case Manager develops partnerships with landlords and community agencies, ensures compliance with HUD regulations and grant requirements, maintains accurate documentation and reporting, and promotes long-term housing stability and self-sufficiency.

Position Details:
  • Hours: 37.5 hour work week; Monday-Friday 8am-4pm; with a rotational on-call schedule.
  • Salary: $20/hr.
Key Responsibilities:
  • Provide case management and housing stabilization services to individuals with disabilities experiencing homelessness.
  • Assess eligibility, develop individualized service plans, and connect clients with community resources.
  • Help clients secure and maintain safe, affordable housing while ensuring HUD and program compliance.
  • Build partnerships with landlords and community agencies to support successful housing outcomes.
  • Maintain accurate documentation, case records, and required grant reporting.
  • Participate in community collaborations, the program's on-call rotation, and other duties as assigned.

Position Requirements:
  • Bachelor's degree in Human Services or related field or Associate Degree with two years' experience or associate degree equivalent with four years' experience.
  • In-depth knowledge and understanding of the social welfare system.
  • Valid driver's license, clean driving history, and reliable personal vehicle. Proof of insurability may also be required.
  • Current FBI, criminal, and child abuse clearance or the ability and willingness to obtain.
