Description
Rural County Case Manager
This temporary or temp to perm positionfor BSWs or MSWs assesses the housing needs of veterans who are homeless or at risk of becoming homeless in Warren, St. Charles, St. Francois, Franklin, Washington, Lincoln, and Jefferson counties of Missouri.
The case manager will utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability. Case managers develop a treatment plan, coordinate individualized services for housing stability, coordinate individualized services for financial stability, and document all support.
This position is located in St. Charles and is Monday through Friday from 8am-4:30pm.
