Job description

The Healthcare Coordination Specialist II will work with residents to develop appropriate case plans and goals and provide on-going assessment and support. Assists the residents to utilize community resources to meet individual needs. Works with residents to identify their skills and abilities to promote personal growth.
Englewood Apartments provides permanent supportive housing for low-income individuals in Chicago.
PAY: $21.23-23/hour, dependent on experience. This is an on-site position.

DUTIES
  • Meet one on one with residents to obtain specific needs information, via a series of assessments, to create with residents a plan to achieve their goals. Advocate on behalf of residents (public aid, social security, home services, etc.) and make referrals as necessary.
  • Work with team to ensure building standards are met and participate in wellness checks and unit inspection follow-up.
  • Create, plan, and implement a monthly calendar of activities that promote Mercy Housing's program model all consistent with funder expectations and data system requirements.
  • Maintain detailed knowledge of the health care system, key service providers and health care delivery system developments related to the health needs of residents.
  • Collaborate with Resident Services Staff to identify residents most in need of care-coordination and self-management assistance and determine what staff member is most appropriate to provide this assistance.
  • Accurately identify/assess resident assets and needs to connect them effectively to resources and help them set personal goals, while recognizing their knowledge and experience.

MINIMUM QUALIFICATIONS
  • High School Diploma or equivalent.
  • Two (2) years of experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.

PREFERRED QUALIFICATIONS
  • Bachelor's Degree in Healthcare Management, Public Health, or related field.
