• Conduct thorough assessments to identify clients' social, emotional, and environmental challenges, ensuring a holistic understanding of their needs.
• Develop and implement individualized service plans that encompass specific goals, interventions, and progress tracking tailored to each client.
• Provide empathetic counseling and support to individuals and families facing a variety of life challenges, focusing on healing and empowerment.
• Collaborate with clients in creating personalized service plans that connect them to vital community resources such as housing assistance, healthcare, and mental health services.
• Engage in crisis intervention, conducting risk assessments and safety planning to support clients in immediate need.
• Coordinate with community agencies and organizations to ensure comprehensive service integration and advocate for clients’ rights and needs.
• Participate in community outreach initiatives to enhance awareness of our agency's services, fostering connections and trust within Fort Worth.
• Contribute to team collaboration through case conferences, regular meetings, and ongoing professional development.

This role focuses on serving a diverse population of individuals and families encountering various psychosocial difficulties. You will work with clients facing challenges related to trauma, mental health, substance use, and socio-economic factors. Your work is essential in empowering these clients to overcome barriers and improve their quality of life.

• Must possess a Master’s degree in Social Work from an accredited institution.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Strong understanding of social and economic justice issues, community resources, and trauma-informed care principles.
• Proficient in communication, cultural competence, and problem-solving skills to effectively connect with diverse client populations.

• Experience working within a social service agency or similar environment, particularly with vulnerable populations.
• Demonstrated success in case management and advocacy roles that have resulted in positive client outcomes.
• Familiarity with mental health treatment modalities and community resource navigation.
