Job description


Job Summary:
Responsible for performing clinical and professional pharmaceutical work in the review, processing, compounding and dispensing of prescribed medications, drugs, and supplies needed for patient care. Receives and makes phone calls related to patient care and prescription processing.
Job Responsibilities:
  • Completes patient records/charts, including initial patient assessment, medication profiles, care plans, initial opening patient notes and continuing patient progress notes.
  • Reviews and supervises the work completed by pharmacy technicians engaged in all areas of the prescription process including processing, mixing/compounding, labeling, packaging, and claim submission of prescriptions and pharmaceuticals/supplies.
  • Completes patient counseling in compliance with clinical program protocols and adhere to and counsel patients in compliance with REMS programs; documents any and all interactions with patients and prescribers in the appropriate systems according to protocol; answers questions and provides information to patients on drug interactions, side effects, dosage, administration and storage of prescriptions and supplies.
  • Reviews patient demographics and prescriptions to assure appropriate therapy for the patient, accuracy of dose, and determines formulas, ingredients, and medications/administration supplies needed for optimal patient outcomes; process, prepare, fill and compound medications, using standard formulas and processes such as weighing, measuring, mixing ingredients, and sterile aseptic preparation of IV solutions where applicable.
  • Follows established procedures concerning security of controlled substances, prescriptions, and supplies, disposal of hazardous waste materials, and security and disposal of aseptic compounding, and quality assurance, where applicable.
  • Participates in the coordination of care to meet patient outcomes/goals including, initial referral, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services.
  • Other responsibilities as judgment or necessity dictate.

External Basic Qualifications
  • BS in Pharmacy or Pharmacist Degree from an accredited educational institution
  • Current Pharmacist license as granted by the appropriate state licensing authority
  • Experience performing prescription dispensing activities, demonstrating a strong working knowledge of applicable state and federal controlled substance laws
  • 2 years' work experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction
  • Excellent written/verbal communication skills and interpersonal skills with the ability to lead, motivate, coach and provide feedback to team members.

Preferred Qualifications
  • 1 year work experience as a practicing pharmacist in a call center environment, mail service, long term care, or specialty pharmacy setting
  • Strong attention to accuracy, detail and quality with proven problem-solving skills
  • Basic skills in Microsoft Office suite of products
