Position Summary

Medication Therapy Management supports the company vision to help people on their path to better health by performing MTM for members at the highest-risk of having medication related problems. The team of pharmacists, technicians, and coordinators work together to identify problems and coordinate care to facilitate resolution to ensure the member is on an optimized medication regimen to help them reach their health-related goals. The staff is trained to use the innovative skill of behavioral modification and interviewing, which is a key driver to positively impact member health outcomes. The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM). Effective delivery of this CVS Health telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers. The MTM Pharmacist interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health-related goals and help them on their path to better health. Pharmacists will also be trained to use the innovative skill of behavioral modification and interviewing, which are key drivers to positively impact patient health outcomes.

Responsibilities:

Perform telephonic comprehensive medication reviews, which includes but is not limited to ensuring a complete medication reconciliation is completed and performing clinical assessments on applicable conditions for each targeted member. You are responsible for engaging and HIPAA authenticating members to a CMR (comprehensive medication review). The annual medication review consists of updating member mailing addresses, documenting medication allergies and side effects, and a full medication list verification of all prescriptions, vitamins, supplements and over-the-counter medications. Identify, document, and facilitate resolution of identified medication related problems. Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy. Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems. Build and maintain positive relationships with internal and external customers including physicians and nurses. Other responsibilities as directed by supervisor and/or manager. You will complete internal training designed to utilize behavioral modification and interviewing. This role will utilize these skill sets as key drivers to positively impact patient engagement and CMR completion.

Required Qualifications

  • Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.

  • 7 Months within the MTM Ops department and Meets Expectations in all core metrics.

  • Previous experience in a call center environment

  • Experience maintaining focus and meeting goals in a work from home environment.

  • Excellent spoken and written communication skills

  • Basic computer skills and proficiency with Microsoft Office products.

  • Exceptional prioritization and organizational skills

  • Acts with integrity and uses sound judgment in dealing with confidential information

  • Applicant must reside and have an active RPh license in the state of PA, IL or FL.

Education

  • BS in Pharmacy or PharmD

  • Active pharmacist license in good standing

The typical pay range for this role is:

$50.48 - $84.62

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
