Essential Functions:

1. Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.

2. Communicate necessary information to others as appropriate.

3. Directly assists patients with mobility, hygiene and activities of daily living. This includes assisting with transportation within the facility and to and from appointments.

4. Assists with patient admissions and discharges

5. Assists nurses and physicians with medical procedures as directed.

6. Provides basic life support in emergency situations.

7. Accurately obtains, documents and reports vital signs / intake and outputs / weights.

8. Monitors general condition of patients and reports observations and significant changes.

9. Recognizes and addresses safety issues

10. Is observant of universal precautions being followed.

11. Has knowledge of, understands and observes special precautions.

12. Maintains a clean and organized environment in patient and staff areas.

13. Communicates courteously and effectively with patients, families and staff.

14. Is protective of patient's rights to privacy and dignity.

15. Displays courteous and professional interactions with the general public, patients, families and staff.

16. Participates in continuing educational activities.

17. Documents, following facility protocols, within the scope of a CNA job description, accurately and completely.

18. Works harmoniously with all members of the Interdisciplinary Team to meet all patient needs.

19. Seeks assistance from Charge Nurse or DNS in any situation that is not within own ability or scope of practice of if there are questions or doubts.

20. Correctly operates all equipment following facility policies and procedures, i.e., telemetry, LifePak12, EKG, Bladder Scan, and Glucometer

21. Completes proper specimen collection (labeling, dating, timing and storage) and delivers to Lab for processing.

22. Follows stocking of supplies and reorder procedure

23. Follows patient transfer procedure for Hospital and ER, to include appropriate records, consents and COBRA.

General Duties:

1. Maintains patient/resident/employee privacy and confidentiality at all times.

2. Other job related duties as assigned.

Wage starts at $20.00
