Eden Vista Hoffman Estates, part of the Vista Senior Living Management Network, is seeking for Wellness Nurse's for our wonderful facility!

Position: Wellness Nurse

Job Description

  • To provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
  • Direct the day-to-day activities of the nursing assistants
  • Prepare and administer medications as ordered by the physician and perform other drug administration.
  • Communicate changes in patient clinical condition with physician, nursing supervisor, and family
  • Complete required record keeping upon residents admission, discharge, and transfer; chart and document per facility policy
Are you:

  • Customer service orientated
  • You must have a current LPN (Licensed Practical Nurse or RN (Registered Nurse) license in good standing.
  • You have a thorough working knowledge of current care standards and regulations
  • You are experienced in the hands-on care of memory-impaired residents.
  • You can maintain and update effective service plans and supervise the care staff.
