Full-Time On-Call Registered Nurse (RN) Hospice

Full-Time On-Call Registered Nurse (RN) Hospice in the Capital Region and Saratoga county areas - Overnights

The Community Hospice is seeking a compassionate and dedicated FT Registered Nurse (RN) to join our After-Hours Hospice Care Team. In this rewarding role, you will provide critical nursing care and support to hospice patients and families in their homes and nursing facilities throughout the Capital Region and Saratoga area during overnight.

Position Summary

  • As an After-Hours Hospice RN, you will respond to patient and family needs during overnight hours, providing skilled nursing assessments, symptom management, and supportive care in homes and nursing facilities. You will collaborate closely with the hospice care team to ensure continuity of care and promote comfort, dignity, and quality of life.

Key Responsibilities

  • Provide after-hours nursing visits to hospice patients in home and facility settings.
  • Assess and manage symptoms, addressing urgent patient and family needs.
  • Provide education, guidance, and emotional support to patients and caregivers.
  • Coordinate care with the interdisciplinary hospice team.
  • Communicate changes in patient condition and ensure appropriate follow-up. Maintain accurate and timely clinical documentation.

Qualifications Required

  • Current New York State Registered Nurse (RN) license.
  • ASN required. BSN preferred
  • 1 year of RN experience required
  • Valid driver's license and reliable transportation.
  • Strong assessment, communication, and critical thinking skills.

Preferred

  • One to two years of nursing experience in home care.
  • Previous hospice or palliative care experience.
  • Hospice experience is preferred but not required.

Pay Range: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
