Weโ€™re looking for a Hospice RN โ€“ Regional Support to join our team. In this dynamic role, youโ€™ll provide direct care to patients and support to families, while also serving as a floating clinicianโ€”temporarily assigned to different hospice branches based on census needs or staffing gaps. If youโ€™re energized by new settings, adaptable to change, and passionate about purpose-driven care, this role could be a perfect fit.

As a Hospice RN โ€“ Regional Support, You Will:


โ€ข Deliver high-quality, compassionate nursing care for hospice patients
โ€ข Manage and implement individualized plans of care
โ€ข Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
โ€ข Assess and address patientsโ€™ physical, emotional, spiritual, and environmental needs
โ€ข Accurately document all care provided and ensure compliance with hospice protocols
โ€ข Educate families and caregivers on care instructions, medications, and safety
โ€ข Participate in the on-call rotation and provide after-hours care as needed
โ€ข Collaborate with physicians, referral sources, and the care team
โ€ข Assist with referrals, regulatory documentation, and payer compliance
โ€ข Travel to various Gentiva hospice locations as assigned to fill urgent needs

Schedule & Travel:

This role typically follows a Daytime Mondayโ€“Friday schedule, with occasional on-call responsibilities. Travel is required and may require limited overnight stays.

About You:

Qualifications โ€“ What Youโ€™ll Bring:


โ€ข Current, unencumbered RN license in the state of employment
โ€ข Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
โ€ข Current CPR certification
โ€ข Valid driverโ€™s license, auto insurance, and reliable transportation

โ€ข Skilled in clinical assessments and compassionate communication
