Job description

Position Description: Summary

We are seeking a compassionate, engaging, and results-driven Health Coach to support patients throughout their health and wellness journey. This role focuses on building meaningful patient relationships, facilitating sustainable behavior change, and providing ongoing accountability within our medical weight management and hormone optimization programs.
The Health Coach plays a vital role in helping patients translate clinical recommendations into daily habits — supporting long-term success, retention, and overall wellness in alignment with our mission.
Position Responsibilities: • Conduct scheduled coaching visits focused on nutrition guidance, movement, stress management, habit formation, and long-term lifestyle change.

• Build strong relationships with patients by understanding their motivations and challenges, providing personalized guidance and accountability.
• Develop and review weekly SMART goals to support sustainable weight management, metabolic health, hormone balance, and overall wellness.
• Maintain a strong understanding of all Options programs and services to confidently guide patients throughout their journey.
• Collaborate with the Medical Provider and Clinic leadership to communicate patient progress and identify opportunities for additional support.
• Monitor patient progress through body composition analysis.
• Collaborate with the Medical Provider to communicate patient progress, barriers, and opportunities for intervention.
• Encourage program retention through proactive follow-up and ongoing accountability conversations.
• Maintain accurate documentation in the EMR system in compliance with HIPAA and company standards.
• Support front desk and clinic operations as needed to ensure a seamless patient experience.
• Adhere to all HIPAA, OSHA, and company compliance policies.

• Bachelor’s degree in Health Sciences, Nutrition, Exercise Science, Public Health, or related field preferred.
• 1–2+ years of experience in health coaching, wellness coaching, personal training, or a related field.
• Experience working directly with clients or patients in a goal-oriented setting.
Preferred Education and Experience
• Health Coach certification (NBC-HWC, NASM, ACE, or similar).
• Experience in medical weight loss, hormone therapy, healthcare, or fitness environment.
• Familiarity with EMR systems and clinical documentation.

Physical Requirements: • Frequent direct patient interaction in a clinical setting.
• Requires periods of standing, walking, and sitting throughout the day.
• Ability to work at a computer for documentation and scheduling.
• May require occasional support at other clinic locations within the market.
