The Hopi people have over 2,000 years of history in the southwest. You will have the opportunity to visit the oldest, permanently inhabited village in North America. Take the opportunity to enjoy an authentic Hopi meal with guidance at the Hopi Cultural Center. Celebrate this culture by eating traditional Hopi food, visiting the Village of Walpi on First Mesa, the most natural of all of the Hopi Villages. Occupying part of Coconino and Navajo counties, this reservation encompasses more than 1.5 million acres and is made up of 12 villages on three mesas.

About the Facility - Hopi Health Care Center, Polacca, Arizona

The Hopi Health Care Center, located in Polacca, Arizona, was built on the Hopi Reservation and serves approximately 7,000 people from both the Hopi and Navajo tribes. The Service Unit site was selected for its central accessibility, where most residents can reach the Center within 35-40 minutes. The Center is JCAHO-accredited, providing innovative health care services to the area's residents. The ambulatory care setting offers primary and preventative medical services, while the facility offers a 2-bed birthing unit, a 4-bed Medical/Surgical adult/pediatric unit supported by a staff of 12 physicians and 3 physician extenders. Services provided include general family medical care, surgical follow-up, pediatric and obstetric services. An eye clinic, staffed by 2 optometrists, provides refraction and general eye care. Seven dentists are stationed at the Center and at a satellite clinic providing dental services. Major trauma, acute surgical emergencies, and high-risk obstetric patients are transported to Tuba City Indian Medical Hospital, Chinle Hospital, Phoenix Indian Medical Center, or to several other contracted health facilities.

Pharmacist - RESPONSIBILITIES (listed are included but not limited to)
  • Screen patient charts to ensure medication orders are aligned with the chart note as well as lab work, vitals, allergies, etc.
  • Process new medication orders.
  • Fill prescription orders when needed.
  • Check filled prescription orders for accuracy.
  • Educate patients on medication therapy.
  • Provide patient education.
  • Work in a collaborative fashion with the pharmacy team and providers to ensure timely and accurate pharmaceutical services.
  • Answer provider questions regarding drug therapy and question providers when appropriate to ensure the proper medication, in the proper dose and dosage form, is dispensed to the patient.
  • Recognize life-threatening emergencies, initiate and maintain resuscitation measures, and perform other emergency procedures as appropriate to the situation.
  • Recognize adverse reactions to medications, treatments, and procedures and initiate interventions as appropriate to the situation.
  • Answer and screen telephone calls, take refill requests, and input outside prescriptions into the system.
  • Perform controlled substance audits as directed
  • Help put away orders as needed.
  • I.V. admixtures and inpatient drug distribution will be carried out based on patient care needs and following Service Unit policies and procedures.
  • Administer immunizations (certification required)
  • The contractor's employee(s) shall participate in training and competency activities, including:
    • Annual safety and infection control
    • Other mandatory training as assigned
Pharmacist - REQUIREMENTS
  • Must have a degree from a school of pharmacy approved by a state-accrediting agency.
  • Shall possess a current, active, unrestricted license from any of the 50 United States, the Commonwealth of Puerto Rico, or the District of Columbia.
  • Shall maintain continuing education/certification and have a current Basic Life Support (BLS) certification.
  • Must have a minimum of 2 years of experience as a pharmacist within the past 36 months.
