RN Case Manager, Home Health

James River Home Health is actively seeking a compassionate and dedicated Home Health RN Case Manager to support our patients in the Chesterfield area. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience.


Salary Range: $73,000 - $90,000

A brief overview of Responsibilities:

  • Provides skilled nursing care to homebound patients in accordance with the established Plan of Care, developing and implementing individualized treatment plans while monitoring and reporting changes in condition.
  • Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team.
  • Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.

Qualifications:

  • Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • At least one year of home health experience preferred
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available during work hours.
  • OASIS and HCHB experience PLUS!
