Job description

Overview

The Scheduler is responsible for the coordination and scheduling of Amerita and Subcontracted Agency (SUB) nursing visits, approving submitted nursing documents, creating nursing delivery tickets, tracking Plans of Treatments, and managing SUB authorizations and invoices.

Responsibilities

  • Coordinates and tracks regularly scheduled Amerita nursing visits for field and AIS
  • Secures Home Health agency for patient visits when Amerita nurse is unable to provide nursing
  • Communicates patient demos and most recent infusion orders to field nurses
  • Alert field nurses when there is a change or an update to an infusion order
  • Communicates assignment of new patients and primary nursing from Nurse Manager to field staff
  • Confirm authorizations and reauthorizations for nursing visits are secured as needed
  • Provides infusion treatment plans to treating providers and maintains communication log
  • Tracks outside agency visits by month
  • Contacts outside agencies for nursing notes, statements and invoices as needed
  • Maintains files of credentials and insurance documents for outside agencies
  • Collaborates with Intake, Sales and Pharmacy staff to schedule visits and orders
  • Review and approve nursing notes in Alayacare or applicable documentation software
  • Review and approve subcontract billing
  • Triage calls from patients, providers and internal partners regarding scheduled nursing visits
  • Assign patient risk codes for all new patients
  • Assist branch with weekend on call duty
  • Supervisory Responsibility: No

Qualifications

  • High School Diploma/GED or equivalent
  • Basic knowledge and experience in home care preferred
  • Good organizational and time management skills
  • Excellent verbal and written communication skills

**To perform this role will require constant sitting, standing, walking, reaching and typing on a keyboard with fingers, and occasionally crouching, stooping, and bending. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
