About the Role:
LifeCare Coalition Outreach-FFA is looking for a compassionate and dedicated In-Home Support professional to join our team in Modesto, CA. Under the supervision of the director/administrator or assistant director, the In-Home Support Staff (IHSS) provides intensive, trauma-informed support services to foster youth placed in resource family homes under the Intensive Services Foster Care (ISFC) program. This role is designed to support youth with elevated behavioral, emotional, or mental health needs by reinforcing treatment plans, promoting placement stability, and assisting resource parents in delivering a safe, structured, and therapeutic home environment in compliance with California Community Care Licensing (CCL) and FFA regulations. The staff reports to agency desiginated supervisor.
Responsibilities:
  1. Provide direct, one-on-one or small group supervision and support to ISFC youth in the resource family home 
  2. Implement Trauma-Informed Care practices consistent with the Core Practice Model (CPM) and agency standards 
  3. Support and reinforce Needs and Services Plan (NSP) goals and behavioral intervention strategies 
  4. Assist youth with activities of daily living (ADLs) including hygiene, routines, organization, and life skills development 
  5. Provide behavioral coaching and de-escalation using approved crisis intervention techniques 
  6. Support resource parents in maintaining structure, consistency, and therapeutic interventions in the home 
  7. Document all services in accordance with California FFA/ISFC documentation standards, including daily notes, and progress updates 
  8. Participate in Child and Family Team (CFT) meetings and collaborate with social workers, mental health providers, and other team members when needed
  9. Ensure compliance with California Department of Social Services (CDSS), Community Care Licensing (CCL), and Continuum of Care Reform (CCR) requirements 
  10. Transport youth to school, therapy, medical appointments, and community activities as needed 
  11. Maintain awareness of and report mandated incidents (e.g., safety concerns, behavioral escalations, AWOL, etc.) per state guidelines 
  12. Provide crisis support and stabilization during high-risk situations

In-Home Support Staff Skills
  1. Strong understanding of trauma, attachment, and behavioral health needs in foster youth 
  2. Ability to implement structured interventions and maintain boundaries 
  3. Excellent documentation and time management skills (required for compliance) 
  4. Effective communication and collaboration within a multidisciplinary team (CFT model) 
  5. Cultural humility and ability to work with diverse families and communities 
  6. Ability to remain calm and responsive in crisis situations

Qualifications:
  1. High school diploma or equivalent required 
  2. Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred 
    1. AA in related field
  3. Minimum 1–2 years experience working with high-needs youth, foster care, behavioral health, or residential settings strongly preferred 
  4. Knowledge of trauma-informed care, crisis intervention, and behavioral support strategies 
  5. Familiarity with California child welfare system and ISFC model preferred 
  6. Valid California driver’s license, reliable transportation, and proof of insurance 
  7. Must meet FFA/CCL clearance requirements, including: 
    • DOJ/FBI fingerprint clearance 
    • Child Abuse Index clearance 
    • TB test/health screening 
    • CPR/First Aid certification (or willingness to obtain)
 
Physical Requirements: 
  • Ability to actively engage with youth (standing, walking, participating in activities) 
  • Ability to safely transport youth 
  • Ability to respond quickly and appropriately in emergency or crisis situations
